You will need to live in our practice area to register with us, if you are unsure whether you are in our area please contact us.

Register online - please complete all 3 steps (16+ Only)

  1) Fill in the Online registration form, ensuring to provide your NHS number, this can be obtained from your previous GP practice.

  2) Also download the Patient Documents and fill in the required pages (listed on page one)

  3) Email the patient documents and proof of address to contact.w92025@wales.nhs.uk.

• If you take regular medication, please include your repeat prescription form with your application to register.
• We may need to see proof of address for your permanent place of residence for each patient over 16 registering. This needs to be a council tax statement, recent bank statement, utility bill in your own name (mobile phone bills are not accepted), or tenancy agreement. 
• We may also require one form of photographic ID - we will contact you if we do. 
•  If you are a foreign national, please ensure you complete the date you first came to UK and the GP you have previously registered with. If you are registering with a GP in the UK for the first time you need a valid visa (work, student) and have paid the ‘Immigration Health Surcharge’ (IHS). Please send us a copy of your visa and proof that you have paid the ‘Immigration Health Surcharge’ (IHS).

If you do not complete all 3 steps your registration will not be processed. We will contact you via the email address you provide if there is a problem with your registration.

 Alternatively you can collect a registration pack from reception - please collect a pack from reception if you wish to register your child.
